Health & Physical Forms – Action Required
We are excited to announce that Seabury Hall is now using August Schools, a secure and user-friendly platform for managing student health records.
What You Need to Know

Parents will receive a personalized link to your child(ren)’s Health Enrollment Packet via a separate email from August Schools on the afternoon of July 9.

If you have more than one student enrolled at Seabury Hall, you will receive a separate email and link for each student.
Required Documents (for all families, new and returning):

Current physical exam

Immunization records or exemption form
-
Any health action plans (e.g., asthma, diabetes, allergies)
-
TB clearance (required for new students or students without one on file)
Please Note:

Physicals are valid for 12 months from the date of the exam and may be reused if still current.

Students may not be allowed to participate in school activities, including athletics and off-campus events, until all required forms are submitted.

Medication Drop off Day
If your student requires medication during the school day, or uses emergency medications (inhaler, EPI Pen, ect), our Medication Drop-Off Day is August 8th from 8:00am - 2:00pm, or by appointment with our School Nurse by emailing nurse@seaburyhall.org
